1. Fill the form out electronically
Open the form in Adobe Acrobat and enter your information electronically by using the Hand tool to select the different fields on the form. Once your data is entered, there are 2 ways to proceed:

1. save the document (the information will not be retained on the form unless the file is saved) and email it to: peer_ctr@peer.berkeley.edu.
   You will need the full version of Acrobat in order to use this option.
2. print the document and fax it to the PEER Outreach Office at 510-231-9471.

2. Fill the form out by hand or typewriter
Print the form and fill it in either by hand or typewriter. Once your information is entered, fax it to the PEER Outreach Office at 510-231-9471.
